Your application must include the following:

• Your name and surname, and signature if the application is written,
• For Turkish citizens: your T.R. identification number; for foreigners: your nationality, passport number, or identity number (if available),
• Your residential or workplace address for official notifications,
• If available, your email address, telephone, and fax numbers for notifications,
• Your request details,
• Any information and documentation related to the subject must be attached to the application if available.
